My next steps

Your next steps

  1. Gather your medicine information

Include prescription medicines, over-the-counter products, vitamins, and supplements. Record what you currently take and bring the list to your care team.

  2. Check the instructions together

Ask your pharmacist or clinician to explain any instructions you do not understand and help resolve differences between your list and your visit summary.

  3. Plan for refills

Write down whom to contact for refills and any questions about cost or getting to the pharmacy.

Questions you can ask

  • What is this medicine for, and how do I take it?
  • What if I miss a dose?
  • Which side effects need a call?
  • Does it interact with anything else I take?
  • Can you help if I cannot afford it?

Let your care team know if cost, transportation, language, supplies, or unclear instructions make a step difficult.
